St. Barnabas Rehabilitation & Continuing Care Center (formerly known as St. Barnabas Nursing Home) is a long-term care facility situated on the ten-acre campus of the acclaimed St. Barnabas Hospital complex in the Belmont section of the Bronx. The Rehabilitation Center has well-maintained grounds with beautiful landscaping and comprises of an attractive, seven-story contemporary building, opened in 1994. Since that time, St. Barnabas Rehabilitation & Continuing Care Center has gained community-wide recognition for its high standards of quality care.

Our mission is to enhance and enrich the lives of the elderly through comprehensive services designed to meet individual needs. We provide a unique environment for long-term care in which residents have the opportunity to develop a sense of control over their lives, maintain their dignity and plan for a fulfilling future within their own capabilities.
In addition to long-term geriatric care, we are renowned for our Adult Day Health Care Program that is available seven days a week. ADHC is a medical model program with services that include door-to-door transportation, breakfast, lunch, recreational activities, wellness programs and community outings.

A new resident entering St. Barnabas Rehabilitation & Continuing Care Center is welcomed into a warm and caring environment. We understand that the transition from home to hospital and then to long-term care may be difficult. With compassion, professional expertise and true concern for individual needs, we are prepared to handle this transition in the most sensitive and dignified manner possible. One of the most important elements in this transition is the continued presence of family members and loved ones.
